Your visit questions, answered

Helpful answers about walk-in care, preparing for your visit, insurance, and payment.

Do I need an appointment?

No appointment is required. Online booking is not offered. Walk-ins are welcome at our four clinics. Call your chosen clinic with questions about your visit or a particular service.

What should I bring to my visit?

Bring a photo ID, your insurance card if you have one, and a current medication list. For a physical, bring the current form your school, sports organization, employer, or examiner requires. For a workplace injury, bring your employer’s authorization and instructions.

Can I bring my child to urgent care?

Call your chosen clinic to confirm whether it can see your child for their age and symptoms. Ask whether a parent or legal guardian needs to attend, and what identification or consent paperwork is required.

How can I find the right clinic?

Our locations are Adrian, Southgate, and Warren in Michigan, and Prospect Urgent Care in Ashtabula, Ohio. Each clinic page has its address, telephone number, and directions.

Can I visit without insurance?

Yes. The clinics currently advertise a $80 self-pay evaluation. Additional services such as laceration repairs, X-rays, injections and breathing treatments carry additional fees. The offer may change. Call with the reason for your visit and ask what the evaluation fee includes, what additional services may cost, and whether the clinic can estimate the total. Your assessment and any recommended services may affect the final cost.

Do you accept Medicaid from another state?

Our Michigan clinics accept Michigan Medicaid plans only. Prospect accepts Ohio Medicaid plans only. Call the clinic with the exact plan name to confirm coverage and network participation.

How do I prepare for a school or sports physical?

Call your chosen clinic to confirm exam availability, age requirements, and parent or guardian attendance. Bring the current form required by your school or sports organization, and check its exam and signature requirements.

Can I get a DOT physical?

Call your clinic to confirm DOT physical pricing and certified examiner availability. Ask what medical records, identification, and current paperwork to bring.

What if I have a question about my bill?

Call the UCM corporate office at 248-957-7999 and choose extension 1 for billing. The office is open Monday–Friday, 10 AM–5 PM, and closed on holidays.

When should I go to the emergency room?

Urgent care is for non-emergency conditions. For a life-threatening emergency, call 911 or go to the nearest emergency department. Severe breathing difficulty, serious chest pain, signs of a stroke, or uncontrolled bleeding require emergency care.
